These Terms of Use (“Terms”) set forth the conditions under which Carmean Family Dentistry (referred to as the “Practice,” “we,” “our,” or “us”) makes this website, located at parkwaydentalcare.com (the “Site”), available to you, including any related services such as scheduling appointments for new or returning patients.

Please review these Terms carefully, as they contain important legal rights and obligations. For clarity, these Terms apply only to your use of the Site and do not govern the provision of dental or healthcare services by the Practice.

BY ACCESSING, BROWSING, SUBMITTING INFORMATION THROUGH, OR OTHERWISE USING THE SITE, YOU AGREE TO BE BOUND BY THESE TERMS. IF YOU DO NOT AGREE, YOU MUST NOT USE THE SITE.

  1. No Dental Services Provided Through the Site

    The content available on this Site is provided solely for informational purposes and to facilitate appointment scheduling. It is not intended to constitute dental, medical, or healthcare advice. Your use of the Site does not establish a dentist–patient relationship with the Practice. The Practice does not, through this Site, recommend or endorse any specific dental treatment, provider, or medication.

  2. Eligibility

    Individuals under the age of 18 may only use the Site with the involvement and consent of a parent or legal guardian who agrees to these Terms. By accessing or supervising use of the Site, you confirm that you are at least 18 years of age and legally capable of entering into binding agreements.

  3. Authorized Use

    The Site and its materials are provided for general informational purposes. The Practice grants you a limited, non-transferable right to access and use the Site solely to learn about our services and to request or schedule appointments.

  4. Restrictions and Ownership Rights

    Message delivery is supported by participating carriers, including but not limited to AT&T, Verizon Wireless, Sprint, T-Mobile, and MetroPCS. Carriers are not responsible for delayed or undelivered messages. Message and data rates may apply. Frequency of messages may vary. For questions regarding your mobile plan, contact your carrier.

  7. Privacy

    Our Privacy Policy is incorporated into these Terms. By using the Site, you consent to the collection and use of information in accordance with that Policy. Information submitted through appointment forms may be collected and used for purposes described in the Privacy Policy, including analytics and marketing, regardless of whether an appointment is completed.

    The Privacy Policy does not address how protected health information (“PHI”) is handled under HIPAA. For information regarding PHI practices, please review our Notice of Privacy Practices.

  8. Reservation of Rights

    We reserve the right to modify, suspend, terminate, or restrict access to the Site at any time without notice. We may monitor usage to ensure compliance, investigate suspected misuse, cooperate with law enforcement, and disclose information when required by law.

Arbitration hearings, if required in person, shall take place in Effingham, Illinois. The arbitration decision shall be final and enforceable in any court of competent jurisdiction. The Practice may seek equitable relief in court where appropriate.

  15. Waiver of Jury Trial and Class Actions

    You and the Practice agree to resolve disputes individually and waive any right to participate in class or representative actions. Arbitration shall be conducted solely on an individual basis.

  16. Governing Law

    These Terms are governed by the laws of the State of Illinois. Courts located in Effingham, Illinois shall have exclusive jurisdiction over disputes not subject to arbitration.

  17. Assignment

    The Practice may transfer its rights and obligations under these Terms without notice.

  18. Survival

    Provisions relating to disclaimers, indemnification, limitation of liability, dispute resolution, and governing law shall survive termination of these Terms.

  19. Entire Agreement

    These Terms, together with the Privacy Policy, constitute the entire agreement regarding Site use. If any provision is deemed unenforceable, the remaining provisions shall remain in effect.

  20. Updates to These Terms
